Zach Braff portrays the character of John 'J.D.' Dorian. J.D. is a young and idealistic medical intern who often daydreams and has an active imagination. Throughout the series, J.D. grows from an inexperienced intern to a skilled doctor while navigating his personal and professional relationships.

Donald Faison plays the role of Christopher Turk, a close friend and roommate of J.D. Turk is also a medical intern and later becomes a skilled surgeon. Turk is known for his sense of humor, love for sports, and his strong bond with J.D.

John C. McGinley portrays the character of Perry Cox, an experienced and highly skilled doctor at Sacred Heart Hospital. Dr. Cox is known for his harsh demeanor, sarcastic wit, and dry sense of humor. Despite his tough exterior, he genuinely cares for his patients and mentees.

Dave Franco plays the role of Cole Aaronson, a cocky and reckless surgical intern at Sacred Heart Hospital. Cole often clashes with his peers due to his arrogant behavior but eventually learns valuable lessons about teamwork and humility.
